Helping with Physical, Emotional and Social Needs

The Mountainside Medical Center Case Management Department, comprised of case managers and social workers, believes that truly comprehensive patient care must emphasize the physical, emotional and social needs of patients – and the loved ones around them. Through our training and experience, and by collaborating with the medical staff and other hospital departments, we are able to help with social-emotional problems that so often go hand in hand with a physical injury or Illness.

Working on the behalf of patients, families, physicians and insurance companies, our Case Management Department team acts as a bridge. We focus on helping people to mobilize and strengthen their own resources to maximize healing, removing obstacles to care whenever possible.

Our staff includes highly experienced RN case managers, social workers and support personnel. Social workers assist with financial details, counseling and discharge to alternative facilities including skilled nursing facilities, rehabilitation facilities, long-term care facilities, hospice and other community programs.

No Mountainside Medical Center patient is denied care based on a third-party determination. In conjunction with your physician, the case manager and/or social worker will work closely with you and your family to identify an alternate care setting that’s right for you.

Utilization Review: Working on Your Behalf to Assure Quality

Insurance regulations and strict federal and state regulations require that all acute care hospitalizations be reviewed to monitor treatment and quality of care. To ensure superior quality, our case managers review each patient’s profile upon admission and throughout the hospital stay. They also provide insurers with the clinical information needed to ensure the level of care is appropriate.

Discharge Planning: Ensuring the Best Possible Continuum of Care

Post-hospital care choices will vary depending on your insurer. The case manager will contact your insurer to determine your specific benefits. A case manager or social worker will meet with you and/or a family member to design a plan for post-hospital care that best meets your needs.
